For a number of years there has been a significant demand for a dedicated 'OK' Health Check for use with children, and in response to that a checklist was produced with the help of a number of practitioners. Unfortunately it was never possible to subject the checklist to the same level of rigorous research as the adult checklist and so it was decided that it would not be proper to make it available commercially. If you wish to use the 'OK' Health Check for Children, you are welcome to do so. You may download it as a pdf file via the following link, and photocopy it for your use. If you would like to submit constructive suggestions for developing the checklist please do not hesitate to send them to the address/email on the 'contact' page. customer base we have produced this free 'OKDATA' file which will allow
users of The ‘OK’ Health Check to build a computer record of the information
gained from client assessment. This is the basic statistical information
a service needs at its fingertips, based on assessment, telling the managers how
many clients have physical disability, how many have epilepsy, how many do not
have input from a physiotherapist and so on.
We
are aware that most organisations already have the Microsoft Office programs
installed on their systems, and so we are making available a very simple Excel
spreadsheet template for you to use. It
is a basic, no frills template which allows you to insert the data from the OK
Health Check forms, and have statistical information at your fingertips about
the health of your client population. FREE DOWNLOAD - 'OKDATA'Right click below and select 'Save Target As'
